چکیده

Effective Human Reliability Analysis (HRA) requires both a qualitative analysis of potential human errors and a quantitative assessment of the likelihood of those errors. One of the main conclusions from an International HRA Empirical Study is the importance of qualitative analysis when performing HRA. Although qualitative data collection is relatively well established for HRA in the nuclear industry, there is very little written guidance available on how to perform such data collection. Most HRA methods do not provide guidance on how to do this, even when the method specifies that this activity should be performed. In addition, HRA is still a relatively new concept in the petroleum industry and so there exists little experience in this industry of qualitative data collection for the purposes of HRA and quantification. The Petro-HRA project is funded by the Research Council of Norway and includes a workpackage to evaluate methods for qualitative data collection, with the aim of developing written guidelines for HRA analysts working in the petroleum industry. This paper describes the objectives and research approach for this workpackage, and the findings to date from interviews with HRA analysts working in the petroleum industry on the Norwegian Continental Shelf.

The strong interconnection and interrelation of safety and security properties of industrial system which are based on programmable logic (field programmable gate arrays, FPGA) is reviewed. Information security, i.e. system's ability to protect the information and data from unauthorized access and modification, is a subordinate property with respect to safety of many instrumentation and control...

As part of comprehensive efforts to develop physics-based risk assessment techniques for space systems at NASA, coupled computational fluid and rigid body dynamic simulations were carried out to investigate the flow mechanisms that accelerate tank fragments in bursting pressurized vessels. Simulations of several configurations were compared to analyses based on the industry-standard Baker explo...

Probabilistic Safety Assessment and Management PSAM 12, June 2014, Honolulu, Hawaii USING SUBSET SIMULATION TO QUANTIFY STAKEHOLDER CONTRIBUTION TO RUNWAY OVERRUN

This paper studies the use of sensitivities to quantify the extent to which individual airline departments (stakeholders) contribute to the incident of runway overrun. For that purpose, we present a model of the incident runway overrun. The incident model is based on the dynamics of aircraft and describes the functional relationship between contributing factors leading to the incident. The inci...

Probabilistic Safety Assessment and Management PSAM 12, June 2014, Honolulu, Hawaii Degradation Modeling and Algorithm for On-line System Health Management using Dynamic Hybrid Bayesian Network

This paper presents a new modeling method and computational algorithm for reliability inference with dynamic hybrid Bayesian network. It features a component-based algorithm and structure to represent complex engineering systems characterized by discrete functional states (including degraded states), and models of underlying physics of failure, with continuous variables. The methodology is desi...

Probabilistic Safety Assessment and Management PSAM 12, June 2014, Honolulu, Hawaii Earthquake Risk Perception: The Case of Mexico City

Given the concerns of society in relation to natural hazards, nowadays the analysis of risk perception and communication play an important role in decision making of those in charge, for example, of Civil Protection. The analysis of risk perception and communication may be regarded not only as a presentation of the scientific calculations of risk, but also a forum for discussion on issues on br...
